1. Who we are
myBJJ Miyazawa is the trading name of Toshihide Miyazawa, a sole trader, ABN 63 877 542 564, of 372 Pennant Hills Road, Carlingford NSW 2118. In this policy, "we" and "the academy" mean that business.
You can reach us at info@mybjjmiyazawa.com.au or on 0422 167 115.
2. What we collect
We collect three kinds of information, and nothing else.
When you enquire through this website. The booking form asks for your name, email address, phone number and an optional message. That is all it sends.
When you train. Before a first class you complete a paper health form and waiver at the desk. It asks for your name and contact details, an emergency contact, and any injury, medical condition or medication we would need to know about in an emergency. For anyone under 18 it also asks for a parent or guardian's details and signature.
When you pay. Membership is paid by bank transfer or in cash at the academy. We do not take card payments, we use no payment processor, and we never see or store card details. A bank transfer shows us only what your bank shows on a statement.
3. Why we collect it
Enquiry details are used to reply to you and arrange a class. Nothing more — we do not run a newsletter and we will not add you to a mailing list unless you ask.
Contact and emergency details are used to run the academy and to reach someone if you are hurt while training.
Health information is used only to keep you safe on the mat: to adapt what we ask of you, and to tell paramedics what they need to know in an emergency. Health information is sensitive information under the Privacy Act 1988 (Cth). We collect it only with your consent, we do not use it for anything else, and we do not disclose it to anyone except emergency services when it is needed to treat you.
4. Who we share it with
We do not sell your information and we do not share it for marketing. The only third parties involved are these, and they are involved because of how the website works.
- Formspree delivers this website's booking form to our inbox. What you type into that form passes through their servers, which are in the United States, so that information leaves Australia.
- Google Fonts serves the typefaces the site uses. Loading a page sends your IP address to Google.
- OpenStreetMap provides the embedded map. Loading a page that shows the map sends your IP address to them.
We use no gym management software. Paper forms are held at the academy and are not uploaded anywhere.
5. Children's information
Children from age 5 train here, in the same class as adults. A parent or guardian provides and signs for a child's information, and can ask to see it, correct it or withdraw it at any time by speaking to us.
Toshihide Miyazawa holds a current New South Wales Working With Children Check.
We photograph classes from time to time. Photographs of anyone under 18 are published only with a parent or guardian's permission, and we will remove any photograph on request — just tell us.
6. Cookies and analytics
This website sets no advertising cookies and no tracking cookies, and runs no analytics. We do not know who visits, or how many.
The site stores exactly one thing in your browser: whether you chose the light or dark theme, so the page looks the same next time. It never leaves your device, and clearing your browser data removes it.
7. How long we keep it, and how it's secured
Email enquiries are kept while we are in contact with you and deleted once they are clearly no longer needed. Signed waivers and health forms are kept while you are a member and for seven years after your last class, because an injury claim can be brought well after the event.
Paper records are held at the academy and are not left where members or visitors can read them. Email is protected by the account's own password and sign-in security. No system is perfectly secure, and we will tell you promptly if something happens to your information that is likely to cause you harm.
8. Accessing or correcting your information
You can ask what we hold about you, ask us to correct it, or ask us to delete it. Email info@mybjjmiyazawa.com.au or speak to Toshi at the academy. We will respond within 30 days, and there is no charge.
We may need to keep a signed waiver even after a deletion request, for the reason given in section 7. If so, we will tell you what we have kept and why.
9. Complaints
If you are unhappy with how we have handled your information, tell us first — info@mybjjmiyazawa.com.au — and we will try to sort it out.
If you are not satisfied with our response, you can complain to the Office of the Australian Information Commissioner at oaic.gov.au or on 1300 363 992.
10. Changes to this policy
If this policy changes we will update this page and change the date below. If a change materially affects how we handle information we already hold about you, we will tell you directly rather than rely on you noticing.
